Case Management
Options Case Managers use the evidence-based Strengths treatment model to help each individual recognize and utilize his or her unique strengths, abilities and potential. Case Managers guide individuals in obtaining the resources that will help them attain important personal life goals such as employment, education, housing, social supports and medical services.
The Strengths Model of Case Management has a proven track record of effectiveness, incorporating values that build on each individual’s strengths, abilities and history of success. The Case Manager and participant work together to map out a plan that will assist the individual in reaching his or her own goals. Strengths Case Managers work with individuals to help them identify and reach goals that are important to them such as employment, education, housing, relationships and healthy living. Case Managers work with each individual in the community wherever it is needed including the home, work, school or other locations.
